Power of atternory (poa) grandmother and alzhiemers?

Ok so I been living with my gma for the past 3 yrs. I have been unemployed for 1 1/2 and still looking for fulltime ( I work part time). I have a 9 month old daughter I do everything by myself for ( dad walked away). Well my gma made me poa when I was like 19 no knowing what that meant the time has come and alzhiemers has started taking control. I am so lost as she handled all her affairs. Like I said I live with her in a apartment I convinced her to get after her home foreclosed after my dads accident ( he's dead). We had a peaceful life and its a win for the both of to enjoy each others company. I don't have good credit at all the apartment told me that if she broke her lease the poa would be responible well that's me. I don't work and I an truly dedicated to school. Can I add my name to the lease to transfer it to a cheaper apartment in the event I can't care for my gma at home? Since I would be deemed responoible for the lease. I really am worry beside I have no place to live without my gma. I don't wanna use her credit I just make sure I am ok so I can focus on getting the best care possible.
